There is definitely something unique about your specific machine... although I can't say whether it is the RGB mod, or something else (because I don't know the details about your system).
Is it possible that there were other modifications done to it, such as PAL (50Hz) modification ? For something like that, even if it is placed into an override state where it's running at 60Hz, there may be some odd delay, desynchronizing something. Wonder Momo's initializtion is very sensitive to exactly how much processing is done by the CPU between system startup and the first VSYNC (or HSYNC) interrupt. This includes wait states on CPU accesses to video memory through the 6270 (which Mednafen has not yet modelled correctly).
Also, if the sync signals are being generated by something other than the Hu6260, this can happen (but this likely would cause other issues in other games too).

No, just RGB with sync and a recap of the system nothing else. Sync comes from the console.

Thank you very much for the wonderful OS elmer. I do have a bug to report. When playing certain CD-Rom2 games using your os and a Super System card rom, some games crash and have random issues like skipping audio. When these games are booted up using a normal 2.0 card rom they play fine. The two games this occurred was the the North American versions of It Came From the Desert and Last Alert.

What happens when you run the System Card 2 ROM image on your TED2? Do these games work then, or do you still get the crashing and skipping audio? I don't actually have either of these games ... can anyone else confirm these problems? Crashing with TEOS (or the older patched Super System Card) is something that I can understand, because the Super System Card area is RAM, and can be corrupted by a game. Skipping audio is a possible symptom, but IMHO it is far more likely to be a power supply issue because of the TED2, rather than a game corrupting the a few specific bytes without actually crashing the console.

There's not a lot of information here... What hardware are you using ? Turbo Duo ? Core Grafx & CDROM ? White PC Engine & UperGrafx ? Core Grafx & SSDS3 ? SuperGrafx and something ? Are you using original CD's or some sort of image ?
Which version of the CDROM Super System Card are you using - Japanese ? Amercian ? One that's altered from the original in some way ?
How far into the game do you notice these issues ?
In order to fix (or even isolate) an issue, one must be able to reproduce it... but there's not enough information here to do that.

I tried the latest version of the unofficial Everdrive firmware (chain-loaded from the original firmware like recommended) but was unable to copy the save file from BRAM to the SD card. A message said that it failed or something like that. Not sure if I did something wrong. The micro-SD card had some problem and was repaired by Windows earlier and I also reformatted it before I tried this. My system is a PC-Engine Duo-R, no modifications.

I expect that TEOS could use some better error-reporting ... but there's not much that I can do without knowing what that error message was. If you want to help find out what went wrong, we can try to debug this through private-messages, or you can mail me that SD card and I can find out exactly what the problem was. But the first thing for you to check is that you recreated all of the backup files in the /TBED/ directory after that reformat, because TEOS won't recreate them itself, it expects them to already exist so that it can just overwrite the data within the file.

I also suspected that might be the issue.

Well I extracted the content of the "teos-beta5-2021-01-26.zip" and put it on the SD-card root after formatting so all the files are fresh. I checked the card again now, and I see that I placed all the content (the two TEOS ROMs and the TBED directory with the backup files) in their own sub-directory on root. I didn't realize the contents of the TBED directory in the zip archive was supposed to go in the regular firmware's directory (also called TBED). Sorry about that.
I fixed it and now saving works and I also confirmed that my saved game loads fine in Mednafen!
Good timing as my trusty Save-kun currently has data corruption issues. I think there's an issue with the cable or something because it's acting funny in Private Eyedol's save manager at times.
Anyway thanks a lot for your time both of you!
